A Comparative Clinical Study Of Onset, Efficacy, Duration Of Action Of Tramadol And Pethidine In Abolition Of Shivering Under Spinal Anaesthesia

Abstract :

Introduction: Spinal anaesthesia is widely used as a safe anaesthetic technique for both elective and emergency operations. Shivering is known to be a frequent complication, reported in 40 to 70% of patients undergoing surgery under regional anaesthesia. The present study was undertaken to study the efficacy of tramadol and pethidine in abolition of shivering and level of sedation under spinal anaesthesia. Methods: The present study was undertaken in Santosh Hospital, Bangalore from Jan 2020 to June 2021. 126 patients belonging to ASA grade I and II, aged between 20-50 years undergoing surgeries under spinal anaesthesia and developed shivering were enrolled in study. 63 in each group (Group P and T) by random division. The drug was administered by another anaesthetic personnel who was blinded to whether the drug contains pethidine or tramadol. Patients were observed at intervals of 1 min till 5 minutes, and thereafter at 10, 20, 30, 45 and 60 minutes for hemodynamic parameters, shivering and sedation. Results: Higher grades of shivering were observed in Group P compared to Group T. This difference was statistically significant at the intervals of 5th, 10th and 30th min. All the study participants in Group T stopped shivering within %mins of administration of Tramadol. But it took up to 60mins for all the participants in Group P to stop shivering. All study participants in Group T were in Grade 2 sedation. Whereas around 12% in Group P were in Grade 3. This difference was even statistically significant. Conclusion: The anti-shivering effect of Tramadol is early and more effective when compared to Pethidine. The sedative effect was less with Tramadol in comparison to Pethidine.
